For many people, the day doesn't truly begin until they've had their first cup of coffee. Some even drink it before eating breakfast, but is that habit doing more harm than good?
According to registered dietitian Anthony DiMarino, probably not. Despite common concerns, most people's digestive systems can handle coffee just fine, even on an empty stomach.
Does Coffee Really Cause Stomach Issues?
There's a lot of debate about whether drinking coffee first thing in the morning leads to digestive problems. Let's break down two of the most common myths.
Myth 1: Coffee Triggers Acid Reflux and Heartburn
Many believe that coffee worsens acid reflux because caffeine increases stomach acid production and relaxes the valve that keeps acid from rising. While this is true to some extent, research suggests that factors like obesity and certain medical conditions have a much bigger impact on acid reflux than coffee does.
Solution: If coffee gives you heartburn, try adding low-fat or nonfat milk to help neutralize its acidity.
Myth 2: Coffee Causes Stomach Ulcers
Stomach ulcers are often blamed on coffee, spicy foods, or stress, but the real culprits are usually a bacterial infection (H. pylori) or excessive use of painkillers like aspirin and ibuprofen. Coffee alone won't cause ulcers, but if you already have one, its acidity might make symptoms worse.
What Are the Side Effects of Coffee on an Empty Stomach?
While coffee itself isn't harmful, drinking it before eating can cause certain side effects in some people.
Increased Jitters and Anxiety: Without food to slow caffeine absorption, coffee hits your system faster, which can lead to shakiness, restlessness, or even heart palpitations. If this happens, eat something before or with your coffee.
Digestive Discomfort: Coffee stimulates bowel movements, which can be inconvenient if you’re rushing out the door. While this isn’t necessarily bad, it’s something to be aware of.

Whole grains (oatmeal, whole-wheat toast) – Helps absorb stomach acid
Healthy fats (avocados, nuts) – Reduces inflammation
Protein (eggs, yogurt) – Slows caffeine absorption
